Abstract

Objective: To evaluate the effectiveness of a clinical pharmacy intervention program in enhancing the safety and rationality of outpatient prescriptions combining chemical and herbal medicines in Bac Lieu province, and to find out factors associated with Drug-Related Problems (DRPs). Subject and Method: A cross-sectional descriptive study with a pre- and post-intervention comparison design was conducted at three healthcare facilities. The effectiveness of the intervention was evaluated by comparing a pre- and post-intervention DRP indicators. Results: After the intervention, the proportion of prescriptions with at least one DRP decreased from 71.7% to 67.0% (p = 0.019); the average number of DRPs per prescription significantly decreased from 1.82 to 1.27 (p<0.001). DRPs related to drug selection, dosage, interactions, and timing of administration were all significantly reduced. Multivariable logistic regression analysis showed that the intervention helped reduce the risk of DRPs (OR = 0.81, 95%CI: 0.66 - 1.00, p=0.045). Other factors influencing DRPs included healthcare facility, patient age, number of examining physicians, physician age and seniority, number of drugs, and number of co-morbidities (p<0.05). Conclusion: Clinical pharmacy intervention plays a crucial role in improving the quality of prescribing combined chemical and herbal medicines. It is necessary to continue refining intervention methods to better control the identified risk factors.
