Characteristics of open gingival embrasures in the maxillary anterior region and associated factors

Abstract
Objective: To describe clinical characteristics and evaluate factors associated with open gingival embrasure area in the maxillary anterior region. Subject and method: This analytical cross-sectional study included 32 patients with 106 interdental sites. Embrasure area, papilla height, and average crown width/height ratio of adjacent teeth were measured on standardized photographs using ImageJ; contact point–alveolar crest distance was measured on CBCT. Data were analyzed using cluster-adjusted OLS regression and Spearman correlation. Result: Contact point–alveolar crest distance showed a moderate positive correlation with embrasure area (rs = 0.570; p<0.001). Both variables differed significantly among interdental sites, with the highest values at 11 - 21. Papilla height, average crown width/height ratio of adjacent teeth, and gingival phenotype were not significantly associated with embrasure area. Conclusion: Contact point-alveolar crest distance showed the most prominent association with embrasure area among the variables examined. The 11 - 21 site should be carefully considered when assessing esthetic risk.
