A case report of nocardia keratitis after smile surgery

Abstract
We report a case of a 22-year-old female patient who presented with a 6-day history of pain, redness, and tearing in the left eye. She was diagnosed with keratitis and treated with topical Levofloxacin 1,5% and Natamycin 5% eyedrops, but the symtoms worsened. She had undergone small incision lenticule extraction (SMILE) refractive surgery 8 days prior to the onset of symptoms. Slit lamp examination revealed a 1,5x1,5-mm central corneal ulcer, a 4x5-mm stromal infiltration with a ring-like pattern and feathery borders in the interface, and hypopyon in the anterior chamber. Scraping of the corneal ulcer was performed and microbiological examination revealed Nocardia. The patient was initially treated with topical Amikacin 5% and oral Biseptol, showing a good response for the first 3 days. However, the infection worsened after epithelialization of the ulcer. The treatment regimen was then changed to topical Meropenem and intravenous Meropenem. The corneal ulcer eventually healed, leaving a corneal scar with neovascularization.
