Treatment outcomes of solar lentigines using q-switched Nd:YAG 532-NM LASER

Abstract

To evaluate the therapeutic outcomes of age spot treatment using a 532-nm Q-switched Nd:YAG laser with artificial intelligence–assisted lesion analysis combined with an intelligent cooling system. Subject and method: twenty-six patients with Fitzpatrick skin types III and IV, presenting with 89 facial lesions diagnosed as age spots, were enrolled and treated at the Aesthetic Center, 108 Military Central Hospital. All lesions were treated in a single session using a Reepot Q-switched 532-nm Nd:YAG laser with a 4-mm spot size, 1-Hz, and energy settings of 200–220 mJ, in combination with an AI-based lesion recognition system and an intelligent skin-cooling technology. Immediately after treatment, the treated areas were covered with a shaped silicone dressing. Treatment outcomes were independently assessed by five dermatologists based on standardized photographs using a 5-grade improvement scale at the time of crust detachment, and at 1 and 3 months post-treatment; Patient pain and satisfaction scores (Numeric rating Scale NRS 0-10; VAS 0-10) during laser treatment. Result: At 12 weeks after a single treatment session, 23 patients (88.46%) achieved excellent pigment improvement (Grade 5), while 3 patients (11.54%) showed good improvement (Grade 3). Procedural pain was minimal, with a mean NRS score of 1.8 ± 0.9, and patient satisfaction was high, with a mean VAS score of 8.9 ± 1.0. Post-inflammatory hyperpigmentation was observed in one patient (3.85%). Conclusion: Artificial intelligence - assisted lesion analysis combined with operative epidermal cooling using a 532-nm Q-switched laser demonstrates promising therapeutic efficacy while minimizing adverse effects in the treatment of age spots among Vietnamese patients.
