Tóm tắt

Mục tiêu: Xác định một số yếu tố có giá trị tiên lượng sớm tử vong nội viện ở bệnh nhân đái tháo đường type 2 có biến chứng tăng đường máu cấp tính. Đối tượng và phương pháp: Nghiên cứu mô tả hồi cứu các bệnh nhân nhập viện do tăng áp lực thẩm thấu hoặc nhiễm toan ceton do đái tháo đường type 2 tại khoa cấp cứu - BV TWQĐ 108 từ năm 2022 – 2025. Xây dựng mô hình hồi quy COX đa biến dựa vào các đặc điểm lâm sàng, cận lâm sàng để xác định các yếu tố dự báo tử vong nội viện. Kết quả: Nghiên cứu bao gồm 107 bệnh nhân đái tháo đường type 2 có tăng áp lực thẩm thấu (72,0%) hoặc nhiễm toan ceton (28,0%) với tỷ lệ tử vong nội viện là 13,1%. Nhóm tử vong có nồng độ urê máu cao hơn, số lượng lymphocyte thấp hơn, nhóm sống sót có ý nghĩa thống kê. Phân tích hồi quy COX đa biến cho thấy RDW (HR = 1,76; KTC 95%: 1,14 - 2,72; p=0,01), PLR (HR = 1,01; KTC 95%: 1,00 - 1,01; p=0,01) liên quan độc lập với nguy cơ tử vong. Tác động nồng độ Urê máu lên nguy cơ tử vong tăng dần theo thời gian nằm viện (p=0,02). Kết luận: RDW, PLR là các yếu tố tiên lượng sớm độc lập tử vong nội viện ở bệnh nhân đái tháo đường type 2 có tăng đường máu cấp tính. Nồng độ Urê máu có giá trị tiên lượng khi thời gian nằm viện kéo dài.
