A multimodal approach to the anatomical study of lumbar artery perforators in the adult Vietnamese population

Abstract

Objective: To investigate the morphology, histology, course, and vascular territory of lumbar artery perforators (LAPs) in adult Vietnamese cadavers, providing an anatomical basis for their application in fasciocutaneous flap design. Subject and method: A descriptive study was conducted on 45 formalin-fixed cadavers (90 lumbar sides), combined with contrast injection on 3 fresh cadavers, Doppler ultrasound in 22 volunteers (44 lumbar sides), and histological analysis of 12 LAP samples. Result: Lumbar artery perforators (LAPs) were most frequently identified at levels LIII - LIV. The mean arterial diameter ranged from 1.12 to 2.54mm, with a wall thickness of 100 - 150µm; the accompanying veins were thinner, measuring 40 - 80µm. The perforators coursed obliquely downward and laterally, traversing the intermuscular septum to reach the thoracolumbar fascia. The thickness of the skin and subcutaneous tissue increased progressively from L1 to L5. The vascular territory was spindle-shaped, with an average length of 72 - 120mm and a width of 35 - 55mm. Conclusion: LAPs exhibit characteristic morphology and histology, with a consistent course and vascular territory, making them reliable pedicles for lumbar fasciocutaneous flaps. These findings support the potential clinical application of LAP flaps in lumbosacral defect coverage and autologous breast reconstruction.
