Clinical and microbiological characteristics of patients with deep neck infections

Abstract

Objective: To describe the clinical, radiological, and microbiological characteristics of patients with deep neck infections treated at the Department of Surgical Intensive Care and Organ Transplantation, 108 Military Central Hospital. Methods: A retrospective descriptive study of 80 patients (2019–2025). Clinical data, CT scan findings, pus and blood culture results, and antibiotic susceptibility patterns were analyzed using SPSS 26.0. Results: The mean age was 68.1 ± 16.4 years, with 55% males. The most common source was odontogenic (42.5%). Abscess formation occurred in 68.7% of cases. The surgical drainage rate was 77.5%, and complications occurred in 23.7% of cases; all patients recovered and were discharged. Comorbidities were present in 70% of patients, most commonly diabetes mellitus and cardiovascular diseases. The rate of positive pus cultures was 62.5%, in which Gram-positive bacteria predominated, with the Streptococcus anginosus group (SAG) being the main pathogen. SAG isolates showed high susceptibility to Linezolid, Vancomycin, and Ampicillin, but high resistance to Erythromycin, Clindamycin, and Tetracycline. Conclusion: Deep neck infections are commonly seen in elderly male patients with underlying comorbidities. Gram-positive bacteria, particularly the Streptococcus anginosus group (SAG), are the predominant pathogens. The combination of appropriate antibiotic therapy, early surgical drainage, and intensive care management helps control the infection and improve patient outcomes.
