Doppler ultrasound characteristics of the right liver graft vasculature in the early phase after living donor liver transplantation

Abstract

Objective: To describe the Doppler ultrasound imaging characteristics of the vascular structures in the right liver graft of patients without graft dysfunction during the early post-operative period following living donor liver transplantation. These findings are correlated with variations in selected blood test parameters. Subject and method: A total of 44 patients underwent right lobe living donor liver transplantation at 108 Military Central Hospital between January 2024 and April 2025. Doppler ultrasound examinations were performed on 1st, 3rd, and 14th post-operative days. Blood test parameters were collected at time points corresponding to each ultrasound examination. Result: Regarding the hepatic artery, the mean peak systolic velocity showed a slight decrease over time, while the resistive index (RI) remained relatively unchanged. One patient (2.3%) exhibited a high-resistance Doppler waveform on post-operative day 3, and another patient (2.3%) demonstrated a Tardus-Parvus waveform on post-operative day 14. Following surgery, the mean flow velocity in the portal vein—both proximal to and at the anastomosis site-tended to gradually decrease. For the right hepatic vein, the mean flow velocity proximal to the anastomosis remained relatively stable, whereas the velocity at the anastomosis site showed an increasing trend. Biochemical parameters including GOT, GPT, total bilirubin, and INR gradually decreased after transplantation. In contrast, GGT tended to increase, while albumin levels remained relatively stable. Conclusion: Doppler ultrasound parameters of graft vasculature in the early post-operative period demonstrated distinct trends and were consistent with the progression of blood test indices in patients without graft dysfunction.
