Clinical and sub-clinical characteristics in acute pancreasitis patients with hypertriglycerdemia at Hai Phong Medical University Hospital in 2024

Abstract
Background: Acute pancreatitis is one of the most common gastrointestinal diseases. The overall mortality rate of it ranges from 1 - 40%. The disease due to increased triglyceride ranks third with atypical clinical course and more severe complications than other causes. In addition, hypertriglyceridemia in patients suffer from Acute pancreatitis has also been shown to increase the rate of local and systemic complications of the disease. Objective: Describe clinical and sub clinical characteristics and comment on some related factors in patients with acute pancreatitis treated at Haiphong Medical University Hospital in 2024. Subject and method: Cross-sectional descriptive study on 40 patients with acute pancreatitis treated as inpatients at Haiphong Medical University Hospital in 2024. Result: Average age 45.6 ± 15.0 years, 80% male. Fatty liver and diabetes are the most common risk factors, accounting for 70% and 50%. 52.5% had very high triglycerides and 25% had normal triglycerides. Common clinical symptoms are abdominal pain (100%), vomiting and nausea, bloating and constipation and abdominal distension. There is a statistically significant difference between CRP and blood cholesterol in 2 groups of patients with and without increased triglycerides with p=0.042, respectively; 0.002. 33.3% of patients had abdominal effusion, 26.7% had pleural effusion; 33.3% had peripancreatic fluid collections; 23.3% had pancreatic necrosis. The average blood amylase was different between the 2 groups (p=0.039). Age and blood triglycerides of acute pancreatitis patients with increased TG have a negative correlation with r = - 0.5 and p=0.004. Conclusion: 52.5% of VTC patients have very hypertriglyceridemia elevation, abdominal pain is a common clinical symptom, there are differences in CRP, Amylase and blood cholesterol in the 2 groups with and without hypertriglyceridemia elevation, there is a correlation. There has relationship between age and hypertriglyceridemia concentration of patients.
