Diagnostic value of SPECT/CT in the detection of parathyroid lesion

Abstract

Objective: This study aimed to evaluate the diagnostic value of 99mTc-MIBI SPECT/CT in comparison with 99mTc-Pertechnetate/99mTc-MIBI dual-tracer scintigraphy and neck ultrasound for detecting parathyroid. Subject and method: The study was conducted on patients suspected parathyroid adenomas underwent parathyroid scintigraphy at the Department of Nuclear Medicine, 108 Military Central Hospital, from March 2024 to March 2025. All patients underwent dual-tracer planar scintigraphy followed by SPECT/CT with MIBI, in accordance with the guidelines of the European Association of Nuclear Medicine (EANM). Result: SPECT/CT achieved a sensitivity of 86.1%, specificity of 92.9%, positive predictive value (PPV) of 94.9%, negative predictive value (NPV) of 81.3%, and an overall accuracy of 88.7%. This modality outperformed planar imaging and ultrasound. Patients with positive SPECT/CT findings had significantly higher serum parathyroid hormone (PTH) and calcium levels (p<0.001 and p=0.025, respectively). The technique was particularly useful in localizing ectopic parathyroid lesions and contributed substantially to preoperative planning. Conclusion: SPECT/CT is a highly accurate method for the diagnosis and localization of parathyroid adenomas.
