Abstract
Objective: To describe the clinical and paraclinical characteristics, and treatment outcomes among hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with peripheral blood eosinophilia. Subject and method: This was a retrospective, descriptive study conducted on 227 adult patients (≥18 years old) admitted for AECOPD at the Emergency Department, 108 Military Central Hospital, from 2022 to 2023. Clinical and laboratory parameters, treatments, and outcomes were measured and compared between patients with and without peripheral blood eosinophilia. Result: A total of 227 patients were included and divided into two groups: An eosinophilic group (n = 82, eosinophil ≥ 2%) and a non-eosinophilic group (n = 145, eosinophil < 2%). The eosinophilic group had significantly lower proportion of antibiotic use (p<0.001), non-invasive ventilation (p=0.024), and invasive mechanical ventilation (p=0.035) compared with the non-eosinophilic group. Median hospital length of stay was 7 days in the eosinophilic group versus 9 days in the non-eosinophilic group (p=0.001). There was an inverse correlation between hospital length of stay and peripheral blood eosinophil count. Patients who died during hospitalization had lower eosinophil counts than survivors (p=0.02). Conclusion: Peripheral blood eosinophilia in patients hospitalized for AECOPD may serve as a useful biomarker for risk stratification and prognosis.
