The performance of NEWS and NEWS-L scoring systems in predicting 30-day mortality in patients with community-acquired pneumonia admitted to the emergency department

Abstract

Objective: To evaluate the prognostic performance of the NEWS and NEWS-L (combined with serum lactate) scoring systems in predicting 30-day mortality in patients with community-acquired pneumonia (CAP) admitted to the Emergency department. These scores including lactate parameter and physiological measurements, which are quickly assessed in Emergency department, open a potential orientation for clinicians when evaluate and prognosis this cohort of patients. Subject and methods: A retrospective cross-sectional study was conducted at the Emergency Department of 108 Military Central Hospital on patients diagnosed with community-acquired pneumonia requiring hospitalization between 2021 and 2022. Clinical and laboratory data were collected. The NEWS and NEWS-L scores were calculated and compared for their ability to predict 30-day mortality. Result: A total of 350 patients aged 18 to 100 years (mean age: 71.73 years) were included. The 30-day mortality rate was 6.3%. The mean scores of NEWS and NEWS-L were significantly higher in the non-survival group compared to the survival group (9.23 ± 3.05 vs. 4.24 ± 3.35 and 11.38 ± 3.37 vs. 6.01 ± 3.99, respectively; p<0.01). At a NEWS cutoff value of 5.5, the sensitivity and specificity for predicting 30-day mortality were 86.4% and 67.4%, respectively, with an AUC of 0.86 (p<0.001). At a NEWS-L cutoff value of 9.6, the sensitivity and specificity were 72.4% and 83.5%, respectively, with an AUC of 0.85 (p<0.001). Conclusion: Both NEWS and NEWS-L scores had good prognostic value for predicting 30-day mortality in patients with CAP.
