Abstract

Objective: This study aimed to evaluate the outcomes of the implementation of antimicrobial prophylaxis procedure with the participantion of the pharmacists in the Department of Orthopedics & Trauma, Duc Giang General Hospital. Subject and method: A prospective study with pharmacist intervention was conducted during the pilot phase (from April to September 2024), the follow-up phase was performed in routine practice with the developed standardized procedure (from October 2024 to March 2025). Information from medical records of eligible orthopedic surgical patients were collected and analyzed. Result: In the pilot phase, 175 patients were included. Overall compliance with antimicrobial prophylaxis procedure was observed on 76.6% of cases, with complete adherence to the choice of antibiotic and the route of administration was obtained. Relative high adherence to antibiotic dosage (95.4%) and the duration of prophylaxis regimens (85.1%) was found. No surgical site infection was reported. Antibiotic cost savings were estimated of 324,000–378,000 VND per patient. In the routine phase, 95.1% of eligible patients received prophylactic antibiotics, with monthly compliance ranging from 67% to 85%. Conclusion: Antimicrobial prophylaxis procedure, supported by clinical pharmacists, enhanced guideline compliance, maintained stable surgical site infection rates, and lower antibiotic-associated costs. This procedure should be maintained and extended for other clean/contaminated-clean surgeries in the hospital.
