Abstract

Objective: To investigate the adverse effects of the bevacizumab–capecitabine regimen used in first-line, second-line, and maintenance therapy for elderly patients with stage IV colorectal cancer (CRC). Subject and method: This was a descriptive case series study involving 48 elderly patients diagnosed with stage IV CRC according to the AJCC 8th edition. All patients received first-line, second-line, or maintenance treatment with the bevacizumab - capecitabine regimen at the 108 Military Central Hospital from January 2022 to May 2025. Result: The mean age of the study population was 73.1 ± 7.4 years, with a predominance of male patients (64.6%). Most patients had good performance status, with 91.7% having an ECOG score of 0 or 1. A total of 75% of patients had metastasis to a single organ, and 64.6% had liver metastases. The most commonly observed adverse events included hand-foot syndrome (43.8%), nausea (37.5%), neutropenia (29.2%), elevated blood urea levels (29.2%), hypertension (25.0%), and thrombocytopenia (25.0%). The majority of toxicities were grade 1 or 2 in severity. Conclusion: The bevacizumab - capecitabine regimen is a suitable therapeutic option for elderly patients with stage IV CRC in first-line, second-line, and maintenance settings, with manageable toxicity profiles.
