Acute appendicitis among united nations peacekeepers at the Vietnamese Level 2 Field Hospital in South Sudan

Abstract
Objective: To evaluate the early outcomes of open appendectomy in the treatment of acute appendicitis (AA) among peacekeeping personnel at the Vietnamese Level-2 Field Hospital in South Sudan. Subject and method: This was a retrospective descriptive study without a control group. A total of 30 patients diagnosed with AA were treated at the Vietnamese Level-2 Field Hospital from May 2021 to May 2025. Result: The mean age was 29.83 ± 5.59 years; 96.67% were male. Military personnel accounted for 83.33%, and civilians for 16.67%. The average appendiceal diameter on ultrasound was 9.36 ± 3.16mm, and wall thickness was 2.47 ± 0.42mm. The appendix was located in the right iliac fossa in 73.33% of cases, subhepatic in 3 cases, and retrocecal in 5 cases (16.67%). Suppurative appendicitis was observed in 16/30 patients (53.34%), and 4 cases had perforated appendix with peritonitis. The average postoperative hospital stay was 6.33 ± 2.75 days. There were two cases (6.67%) of postoperative complications, both being surgical site infections. Pain scores on postoperative day 1, day 2, and at discharge were 5.57 ± 1.28, 3.97 ± 1.16, and 1.42 ± 0.51, respectively. Conclusion: Open appendectomy is an effective and reliable treatment for acute appendicitis in field conditions without access to laparoscopic surgery.
