Survey of relationship between kidney function and hemodynamic indicators of grafted renal arteries in kidney transplant patients

Abstract
Objective: To analyze the relationship between glomerular filtration rate (GFR) and several hemodynamic indicatorrs of the renal allograft arteries in post-transplant patients from living donors. Subject and method: A retrospective and cross-sectional study was conducted on 62 patients who received kidney transplants from living donors between August 2023 and September 2024 at Military Hospital 103 (MH103). Result: The mean estimated glomerular filtration rate (eGFR) of the patients was 59.58 ± 16.01mL/min/1.73m². The graft renal artery at the renal hilum had a peak systolic velocity (Vs) of 102.7 ± 20.60cm/s, resistive index (RI) of 0.66 ± 0.065, pulsatility index (PI) of 1.22 ± 0.32, and acceleration time (AT) of 82.82 ± 29.59ms. Elevated Vs was observed in 17.7% of patients; elevated RI in 1.6%; increased PI in 11.3%; and prolonged AT in 30.6%. There were significant associations between Vs, RI, and PI of the graft renal artery and the post-transplant estimated GFR (p<0.05). The RI of the graft renal artery was positively correlated with serum CRP levels, while the PI was positively correlated with serum urea, creatinine, and CRP levels, and negatively correlated with post-transplant eGFR (p<0.05). Conclusion: There is a relationship between the hemodynamics of the renal allograft artery (RI, PI, Vs) and eGFR in post-transplant patients.
