Investigation of changes in left ventricular longitudinal myocardial strain using speckle-tracking echocardiography in patients with septic shock

Abstract

Objective: To describe the changes in left ventricular longitudinal myocardial function in patients with septic shock treated at the Intensive Care Unit, Military Hospital 175. Subject and method: A prospective descriptive study using convenient sampling was conducted on 56 patients diagnosed with septic shock and treated at the ICU of Military Hospital 175 from January 2023 to December 2024. Result: The mean age was 67.5 ± 15.2 years, with a predominance of male patients (75%) and a continuous renal replacement therapy (CRRT) rate of 55.4%. Escherichia coli (25%) and Klebsiella pneumoniae (21.4%) were the most commonly isolated pathogens, with the urinary (32.2%) and respiratory (25%) tracts being the main infection entry sites. The rate of combination therapy using ≥ 2 antibiotics was 82.1%. At time point T0, the mean global longitudinal strain (GLS) was -14.1 ± 3.1%. All strain indices (GLS, LS-4C, LS-2C, and LS-3C) improved significantly after treatment (p<0.001). Reduced GLS was associated with higher SOFA scores, lactate > 5mmol/L, NT-proBNP > 14,000pg/mL, and vasopressor use (p<0.05). Conclusion: Septic shock predominantly affects older adults, with gram-negative bacteria being the most common pathogens. GLS tends to decrease in septic shock and is closely associated with disease severity and prognostic markers.
