Abstract

Acute pancreatitis associated with hypercalcemia is a rare condition, accounting for approximately 1.5-8% of all cases. Among the various causes of hypercalcemia, primary hyperparathyroidism (Primary hyperparathyroidism - PHPT) is recognized as a leading contributor and has been implicated in the development of acute pancreatitis. The association between PHPT and acute pancreatitis has been noted in medical literature; however, this relationship remains inconclusive due to limited data from large-scale studies. In this case series, we described two cases of recurrent acute pancreatitis with initially unknown etiology, later found to have elevated serum calcium and parathyroid hormone (PTH) levels, leading to the diagnosis of PHPT. Both patients underwent parathyroidectomy, which resulted in normalization of calcium and PTH levels and resolution of pancreatitis recurrence. This case series emphasizes that hypercalcemia should be considered as a potential cause in cases of recurrent idiopathic acute pancreatitis, especially when elevated serum calcium was documented during previous episodes; and highlights PHPT as a potential contributing factor in the disease pathogenesis.
