Evaluation of esophageal squamous cell carcinoma characteristics in patients with head and neck squamous cell carcinoma

Abstract
Objective: To investigate the prevalence and clinical characteristics of esophageal carcinoma in patients with a history of head and neck squamous cell carcinoma (HNSCC). Subject and method: This retrospective study included patients diagnosed with HNSCC and treated at 108 Military Central Hospital from 01/2015 to 05/2025. Medical records were reviewed to identify cases subsequently diagnosed with esophageal carcinoma. Patients were classified into synchronous and metachronous groups based on the timing of esophageal carcinoma diagnosis relative to HNSCC. Result: Among 285 patients with HNSCC, 42 (14.7%) developed esophageal carcinoma. The mean age was 58.8 ± 7.7 years, and all patients were male. Of these, 33.3% were diagnosed with esophageal carcinoma synchronously with HNSCC. The highest incidence of esophageal carcinoma was observed in patients with hypopharyngeal cancer (76.2%). The middle third of the esophagus was the most common site of esophageal carcinoma (81%). No significant differences were found between the synchronous and metachronous groups in terms of age, history of smoking or alcohol consumption, primary tumor location, or esophageal carcinoma location. Conclusion: The prevalence of esophageal carcinoma in patients with HNSCC was high. These findings highlight the need for routine upper gastrointestinal endoscopic screening in patients with HNSCC to facilitate early detection and improve outcomes.
