Assessment of SPARCC scoring and its correlation with clinical and paraclinical parameters in patients with axial spondyloarthritis

Abstract
Objective: To evaluate the SPARCC score and its association with clinical characteristics, laboratory parameters, and disease activity indices in patients with axial spondyloarthritis (axSpA). Subject and method: A cross-sectional descriptive study was conducted on 50 patients diagnosed with axSpA at the 108 Military Central Hospital from September 2023 to April 2025. Clinical features, laboratory data, and sacroiliac joint magnetic resonance imaging (MRI) findings were collected. The SPARCC score was calculated based on the criteria of the Spondyloarthritis Research Consortium of Canada. Result: The mean SPARCC score was 29.28 ± 18.93. Patients with BASDAI ≥ 4 had significantly higher SPARCC scores compared to those with BASDAI < 4 (p<0.05). A weak positive correlation was observed between SPARCC score and ASDAS-CRP (r = 0.29, p=0.04), while a moderate positive correlation was found with BASDAI (r = 0.31, p=0.02). Conclusion: The SPARCC score is a valuable tool in assessing sacroiliitis on MRI in axSpA patients. It shows weak to moderate positive correlations with disease activity indices and may serve as an objective marker to support disease monitoring.
