Abstract

Objective: To evaluate the initial effectiveness of the inpatient medication review process at the Department of Gastrointestinal Diseases. Subjects and Methods: A prospective cross-sectional descriptive study was conducted on all medication orders at the Department of Gastrointestinal Diseases from March 10 to April 7, 2025. Results: A total of 830 medication orders were reviewed for 181 patients. The improved process included assigning a dedicated pharmacist to the department and reviewing medication orders immediately after prescribing, prior to the issuance of dispensing slips. Review time increased from 10 to 120 minutes per day, averaging 5.78 minutes per order. 93.3% of orders were reviewed before being compiled. Pharmacists proposed adjustments to 54 orders (6.5%), mainly due to incorrect drug selection (74.1%) and dosing errors (25.9%). Interventions included drug discontinuation (40.7%) and adding ICD-10 codes (24.1%), with a 94.4% acceptance rate by physicians. Dosage information was the most commonly provided by pharmacists, accounting for 35.1%. Conclusion: The improved medication review process with dedicated pharmacists enhanced medication safety and rational use, reinforcing the professional role of pharmacists in hospital practice.
