Evaluation of miRNA-21-5p and miRNA-145-5p expression in HBV-related hepatocellular carcinoma and chronic hepatitis patients

Abstract

Objective: This study aims to determine the expression levels of miRNA-21-5p and miRNA-145-5p and their diagnostic value in hepatocellular carcinoma (HCC) caused by HBV infection, especially in early-stage HCC in patients with chronic hepatitis B (CHB). Subject and method: This cross-sectional descriptive study included literature review, in silico screening using Mienturnet software and UALCAN database for bioinformatics analysis, and the selection of miRNA-21-5p and miRNA-145-5p, which showed differential expression levels between the HCC group and the CHB group. The study quantified miRNA-21-5p and miRNA-145-5p using qPCR on the serum of 200 patients, including 100 patients with hepatocellular carcinoma (HCC) due to chronic HBV infection and 100 patients with chronic hepatitis B (CHB). Result: The expression level of miRNA-21-5p was significantly upregulated in the HCC group compared to the CHB group (fold-change = 3.9, p<0.001). In contrast, miRNA-145-5p expression was downregulated by 2.06-fold in the HCC group compared to CHB patients with HBV infection. The area under the curve (AUC) and sensitivity of miRNA-21-5p and miRNA-145-5p, when combined with AFP, increased significantly compared to the AUC of AFP alone, both in the HCC group and in early-stage HCC (BCLC-A) compared to the CHB group. Conclusion: miRNA-21-5p and miRNA-145-5p are significant for diagnosing HCC in patients with chronic hepatitis B. When combined with AFP, they may enhance the diagnostic significance of HCC.
