Evaluation of the postoperative pain relief effectiveness of ultrasound-guilded bilateral superficial cervical plexus block with bupivacain combined with dexamethason after thyroidectomy

Abstract

Objective: To evaluate the postoperative analgesic effectiveness of bilateral superficial cervical plexus block (SCPB) guided by ultrasound. Subject and method: From February 2024 to July 2024, 100 patients aged 17 - 72 with thyroid cancer undergoing lobectomy or total thyroidectomy were administered intraoperative and postoperative analgesia using bilateral SCPB with 0.25% bupivacain combined with dexamethason 4mg (8 - 10ml per side), guided by ultrasound. Pain relief efficacy and adverse effects were monitored at 1, 3, 6, 12, and 24 hours post-surgery. Result: The majority of patients experienced good pain relief, with a resting VAS score of ≤3 and an average VAS score during movement of ≤ 4. Only 2 patients had a VAS score of 5. Heart rate, blood pressure, respiratory rate, and SpO2 showed no statistically significant changes compared to preoperative values (p>0.05). No complications such as intravascular injection, local anesthetic toxicity, or phrenic nerve paralysis were observed. Conclusion: Bilateral SCPB using 0.25% bupivacain combined with dexamethason for thyroidectomy provides effective and long-lasting postoperative pain relief with minimal side effects.
