The value of endoscopic ultrasound in diagnosing the invasiveness of early gastric cancer and precancerous lesions

Abstract
Objective: To evaluate the value of endoscopic ultrasound (EUS) in diagnosing the invasiveness of early gastric cancer and precancerous lesions. Subject and method: A total of 35 patients with localized gastric lesions detected through conventional endoscopy at the 108 Military Central Hospital from January 2018 to January 2023 were included. EUS was performed following standardized protocols. Diagnostic outcomes of EUS were compared with pathological findings. Result: The average age of the patients was 58.1 ± 10.7 years. The most common location was the lesser curvature, accounting for 42.9%, followed by the antrum, 28.6%. Most patients had tumors ≤ 2cm in size, accounting for over 80%. Endoscopic ultrasound (EUS) stratification showed that mucosal invasion (M1/2) was the majority, with 57.1%. Diagnosis of stage T1a (mucosal invasion) based on histopathology was predominant at 77.1%. The accuracy of endoscopic ultrasound diagnosis compared with pathological results showed that the highest rate was in the mucosal layer, with 85.7%; decreasing as it entered deeper layers of the muscularis mucosae (82.9%) and submucosa (79.4%). Conclusion: EUS is an effective tool for diagnosing early-stage gastric cancer, providing significant value in selecting appropriate treatment modalities and improving prognosis for patients with early gastric cancer and precancerous lesions
