Abstract
Objective: To assess the feasibility, safety, and efficacy of The DK mini-Culotte (DKmCU) technique for true bifurcation lesions (TBLs). Subject and method: This was a prospective study with a 1-year clinical follow-up. The primary clinical endpoint was major adverse cardiovascular events (MACE). Secondary endpoints included clinically driven target vessel/lesion revascularization (TVR/TLR), stent thrombosis, and in-stent restenosis. Results: 37 consecutive patients with TBLs (Medina classification 1,1,1; 1,0,1; 0,1,1) were included. The mean age was 66.24 ± 10.81 years, and 23 patients (62.2%) were male. The distribution of TBLs was as follows: LMCA-LAD/LCx (35.1%); pLAD-mLAD/D (54.1%); dRCA-PLV/PDA (8.1%); and pLCx-dLCx/OM (2.7%). Initial QCA analysis showed the main vessel lesion length, diameter, and diameter stenosis were 33.27 ± 10.49mm, 3.08 ± 0.40mm, and 81.22 ± 12.50%, respectively, while the side branch lesion length, diameter, and diameter stenosis were 24.78 ± 8.42mm, 2.93 ± 0.40mm, and 86.05 ± 9.90%, respectively. The technique's success rate was 100% with no procedural complications. Conclusion: The DKmCU technique demonstrated a low cumulative MACE rate and TVR/TLR, with efficacy comparable to other techniques in treating TBLs.
