Drug-related problems and associated factors in hospitalized patients with chronic kidney disease at a provincial hospital in Ho Chi Minh city

Abstract

Objective: To determine drug-related problems (DRPs) occurring in hospitalized patients with chronic kidney disease (CKD). Subject and method: A descriptive cross-sectional study was conducted on medical records of inpatients with CKD (≥ 18 years old) treated at the Nephrology and Cardiology Departments, a provincial hospital in Ho Chi Minh City, from 1st October 2022 to 31th December 2022. DRPs were identified and categorized according to Pharmaceutical Care Network Europe, version 9.1, and investigated the potentially clinical relevance using Doerper scale (2015). DRPs-related factors were identified using multivariate logistic regression model. Result: A total of 140 medical records of patients with CKD was included. The rate of medical records with at least 1 DRP was 76.4%. The most common DRPs related to dosing frequency (35.6%), dosage (31.1%) and timing of administration (20.8%). There were 13.5% DRPs potentially harming patient with significant consensus, ICC = 0.686 (0.555-0.788), p<0.001. Patients with ≥ 5 comorbidities, hospitalization ≥ 5 days were associated with a higher incidence of DRPs than those with fewer comorbidities or shorter hospital stay (p<0.01). Conclusion: The rate of hospitalized patients with CKD encountering DRPs was quite high. Appropriate interventions should be implemented, especially in patients with multiple comorbidities and long hospital stay to minimize the occurrence of DRPs.
