Abstract

Objective: To compare the anesthetic effectiveness of interscalen brachial plexus anesthesia under the guidance of ultrasound using a mixture of bupivacaine - dexamethasone and bupivacaine for upper limb surgeries. Subject and method: Prospective comparative study, 60 patients aged 16 to 70, ASA I, II, randomly divided into 2 groups, group 1: 30 patients using a mixture of bupivacaine (1.5mg/kg) - dexamethasone (8mg). Group 2: 30 patients using bupivacaine (1.5mg/kg) received brachial plexus anesthesia for upper limb surgery at 108 Military Central Hospital. Result: The onset time of sensory block group 1 (4.73 ± 0.98 minutes) was shorter than group 2 (10.10 ± 2.78 minutes) with p<0.05. The duration time of sensory block group 1 (1049.77 ± 258.12 minutes), was longer than group 2 (553.83 ± 228.06 minutes), with p<0.05. The onset time of motor block group 1 (8.03 ± 1.87 minutes) was shorter than that of group 2 (17.57 ± 5.54 minutes) with p<0.05. The duration time of motor block group 1 (1025.60 ± 258.86 minutes) was longer than group 2 (470.97 ± 182.92 minutes) with p<0.05. Heart rate, blood pressure, breathing rate, SpO2 were stable and there was no difference between the 2 groups with p>0.05. Conclusion: Anesthesia of the interscalene brachial plexus under the guidance of ultrasound with a mixture of bupivacaine - dexamethasone is a method of shortening the onset time and prolonging the duration time of sensory block and motor block compared to bupivacaine alone.
