Abstract

Primary hyperparathyroidism is a relatively common endocrine disorder mainly caused by parathyroid adenoma. However, some ectopic parathyroid adenomas are sometimes challenging to diagnose. We report a case of a 64-year-old man with primary hyperparathyroidism caused by an ectopic parathyroid adenoma in the anterior mediastinum. The patient had very severe initial clinical manifestations due to hyperosmolarity caused by polyuria and hypercalcemia. Imaging tests, including parathyroid scintigraphy and CT neck and chest scans, help diagnose the tumor's ectopic state. After surgical treatment, the patient recovered completely.
