Research on some right ventricular morphological characteristics and longitudinal strain in patients with acute myocardial infarction

Abstract
Objective: To investigate some right ventricular morphological characteristics (RV base, RV mid, RV length and RV free-wall thickness) and right-ventricular longitudinal strain in patients with acute myocardial infarction. Subject and method: 95 patients with acute myocardial infarction were treated at Heart Center of 103 Military Hospital, from March 2023 to January 2024. Research variables were measured according to ASE guidelines 2015. Result: Mean age: 68.25 ± 12.07, 70.59% was male; 73.68% of patients was STEMI. The mean RVD1, RVD2, RVD3 and RV free wall thickness were 27.82 ± 5.71mm; 21.15 ± 4.88mm; 57.92 ± 11.27mm; 5.68 (5.35-6.88) mm, respectively. The mean of RVFWSL and RV4CSL were -17.82 ± 8.41% and -14.57 ± 6.82%, respectively. RVD2 in RCA culprit lesion group was higher than other groups (p<0.05). RV longitudinal strain decreased significantly in STEMI group compared with NSTEMI group (RVFWSL: p=0.02; RV4CSL: p=0.01); RV longitudinal strain in LAD culprid lesion group was lowest with the quartiles of RVFWSL and RV4CSL were -12.1 (-16.5, -8.1)% and -16.0 (-21.0, -10.1)%, respectively. RV longitudinal strain in LCx culprid lesion group was highest with the quatiles of RVFWSL and RV4CSL were repectively -17.7 (-24.8, -10.6) % and -24.8 (-28.7, -14.7) %. The linear regression analysis showed that RVFWSL and RV4CSL were correlated negatively to EF BP (r=-0.403 and -0.483; p<0.05) and positively to LV (r= 0.473 and 0479, p<0.05). Conclusion: RVD2 in RCA culprit lesion group was higher than other groups. RV longitudinal strain decreased significantly in STEMI group compared with NSTEMI group, RV longitudinal strain in LAD culprit lesion group was lowest, in LCx culprit lesion was highest. RV4CSL and RVFWSL were both correlated negatively to EFBP and positively to LV-GLS
