Evaluation of right hepatic vascular and biliary anatomical variations in 150 cases of right liver graft

Abstract

Objective: Understanding anatomic variations of the right lobe is fundamental in adult living donor liver transplantation. Subject and method: We analysed anatomy in 150 right liver donors from October 2017 to December 2022 at 108 Military Central Hospital. Result: Portal vein: Normal anatomy was found in 94.7% donors. Hepatic artery: Single arterial inflow of the right liver was identified in 148 (98.7%) of donors. Bile duct: Classic anatomy was identified in 84.5% of donors; 4.5% had a trifurcation of the common bile duct; in 2.7% the right anterior and in 5.3% the right posterior bile duct opened into the left bile duct. Hepatic veins: In 40.3% significant accessory hepatic vein (> 5mm) were preserved, in 50,4% the middle hepatic vein was harvested. Conclusion: While anatomic variations in the right liver donor were common, no contraindication to right liver harvesting was noted in this study.
