Abstract

Objective: To evaluate the intraoperative and postoperative results of laparoscopic surgery for
right-sided colon cancer and some related factors. Subject and method: A prospective study on 60 patients with right colon adenocarcinoma who underwent laparoscopic complete mesenteric excition (CME) with high ligation (HL) in the period from 1.2016 to 12.2021 at HCM Oncology Hospital. Evaluation of laparoscopic results, and oncological results. Result: Average operative time 129 minutes, average blood loss 66ml, intraoperative complications significantly related to high ligation of the right colon artery and middle colon artery compared with just resection of the ileocolic vascular (p=0.013), conversion rate to open surgery 0%. There was no postoperative mortality; postoperative complication rate was 5% and was treated conservatively. The average follow-up time was 37.8 (10-84) months, the cancer recurrence rate was 15%; T4a tumors had a significantly higher risk of recurrence compared with T1, 2, 3 (p=0.023). Conclusion: CME+HL laparoscopic right colectomy for adenocarcinoma must be feasible and highly safe. Tumor invasion to the serosa significantly increases the level lymph node metastasis of cancer recurrence.
