Prognostic value of the VASOGRADE scale in predicting delayed cerebral ischemia in patients with aneurysmal subarachnoid hemorrhage

Abstract

Objective: To evaluate the prognostic value of the VASOGRADE scale for delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (SAH). Subject and method: A prospective descriptive study was conducted on 207 SAH patients treated at 108 Military Central Hospital and Bach Mai Hospital from 8/2022 to 7/2023 to assess the relationship between VASOGRADE and DCI.
Result: Patients with VASOGRADE-Yellow had a 4-fold higher risk of developing DCI (odds ratio [OR], 4.48; 95% CI: 1.01-19.97) compared to VASOGRADE-Green; those with VASOGRADE-Red had an 8-fold higher risk of DCI (OR, 8.16; 95% CI: 1.77-37.64) compared to VASOGRADE-Green. The VASOGRADE score had predictive value for DCI with an area under the ROC curve of 0.63, 41.67% sensitivity, and 76.1% specificity. Conclusion: The VASOGRADE score has prognostic value for delayed cerebral ischemia (DCI) in aneurysmal SAH patients. It can help stratify patients at risk of developing DCI, allowing appropriate monitoring and treatment.
