Value of multi-slice computed tomography for the diagnosis of fungal sinusitis

Abstract

Objective: The aims of this study was to evaluate the value of multi-slice computed tomography for the diagnosis of fungal sinusitis. Subject and method: A descriptive study was conducted at Hanoi Medical University Hospital from January 2022 to July 2023 on 70 patients with chronic rhinosinusitis who underwent clinical examination and nasal endoscopy, multi-slice CT scan of the sinuses, then endoscopic sinus surgery and post-operative fungal testing. Sinus lesions on CT were compared with post-operative fungal test results to evaluate the sensitivity (Sn), the specificity (Sp), the positive predictive value (PPV), the negative predictive value (NPV) of CT for fungal sinusitis diagnosis. Result: Fungal sinusitis was diagnosed in 60/70 patients, accounting for 86%, of which 46/60 patients were sinus mycosis, accounting for 76.7%, the remainder were chronic invasive fungal sinusitis. Based on the signs of unilateral sinus lesion, intra-sinus opacities, calcifications in the intra-sinus opacities and condensation/osteolysis lesion of the sinus walls, multi-detector CT had the Sn, the Sp, the PPV and the NPV for the diagnosis of fungal sinusitis were 98.3%, 70%, 95.2% and 87.5%, respectively. Conclusion: Although the sample size was still limited, the study showed that multi-slice CT was a highly valuable non-invasive method for diagnosing fungal sinusitis.
