Abstract

Objective: This study aimed to determine drug-related problems (DRPs) in outpatients’ prescriptions. Subject and method: A descriptive cross-sectional study was carried out to collect prescriptions of outpatients from 1st to 29th, April 2021. DRPs were determined using the Pharmaceutical Care Network Europe (PCNE) classification, version 9.1. The potentially clinical relevance of DRPs was assessed by two experts using the National Coordinating Council for Medication Error Reporting and Prevention (NCC-MERP) classification. DRPs-related factors were identified using multivariate logistic regression model. Result: A total of 1280 prescriptions was included. The median age of patients was 63 (54-70). More than half of the patients were female (57.9%). The total number of DRPs identified was 1594, about 70% of patients had at least 1 DRP. The most common DRPs was inappropriate timing of administration (61.3%), and dosage frequency (12.8%). Other DRPs had low proportions. The two experts considered 7.1% and 5.5% of DRPs was potentially harmful with the level of consensus was substantial (Cohen's kappa = 0.61). Prescriptions of male patients, with ≥ 5 drugs, having class A drugs, class M drugs and class N drugs were more likely to occur DRPs. Patients with diseases musculoskeletal system and connective tissue and having prescriptions indicated by a male physician were less likely to experience DRPs (p<0.05). Conclusion: The proportion of prescriptions with DRP was quite high. Providing information to physicians about common DPRs, especially, potentially harmful DRPs may help reduce DRPs.
