Abstract

Objective: To describe a simple technique of the percutaneous release of the A1 pulley for trigger finger with an 18-gauge needle and to evaluate the result of this method. Subject and method: 350 patients (370 fingers) who being suffered from trigger finger at third stage of the Green classification (1997) [1], were treated by the percutaneous release of the A1 pulley with an 18-gauge at 7A Military Hospital from January 2015 to December 2018. There were 210 women and 140 men. The mean age of the patients was 50 years old (38 - 65 years old). Method: A non-controlled prospective clinical trial. This study evaluates the safety and efficacy of the percutaneous release with an 18-gauge. Patients were examined with a clinical staging for the Green stage at 1 week, 3 months, 6 months after the initial treatment. Result: The mean duration of follow-up was 18 weeks (8 - 24 weeks); 100% had complete release of the trigger finger. 44 cases (12.57%) still had a metacarpophalangeal joint pain after surgery, and these 44 cases were rheumatoid arthritics. These 44 cases required oral anti-pain medication for one week. No complications and relapse were recorded in or after the surgeries. Conclusion: Percutaneous A1 pulley release is an effective, safe, and convenient procedure for the treatment of trigger finger.
