Biliary complications after a right lobe in living donor liver transplantation

Abstract
Objective: To evaluate biliary complications post-operative right hepatectomy in living donor liver transplantation. Subject and method: A retrospective study of 148 cases of donors who underwent right hepatectomy from October 2017 to October 2022 at 108 Military Central Hospital. Result: Biliary anatomical variation: Type I was the most common type with 126 (85.14%) cases; type II: 8 (5.41%) cases; type III: 11 (7.44%) cases; type IV 3 (2.03%) cases. All the 7 cases having biliary complications were in type 1 group: 5 (3.38%) cases of bilary leakage treated with drainage, 4 of which was successful and 1 was unsuccessful and needed reoperation; 02 (1.35%) cases had biliary stenosis treated with stent placement via endoscopic retrograde cholangiopancreatography (ERCP). Based on Clavien-Dindo classification for complications, level IIIa was witnessed in 6 (4.05%) cases, IIIb in 1 (0.68%) case. No cases of mortality in the donors of our study. Conclusion: Biliary complications is the most common challenge post-operative right hepatectomy in ling donor liver transplantation and can be succesfully treated with drainage or ERCP.
