Radiation therapy for locally advanced cervical cancer in the elderly patients

Abstract

Objective: To evaluate the efficacy and toxicity of definitive treatment in elderly patients (≥ 70 years-old) with locally advanced cervical cancer. Subject and method: We reviewed 73 patients with locally advanced cervical cancer (stage IB3, IIA2, IIB-IVA) treated by curative intent from May 2018 to April 2019 at Hochiminh city Oncology Hospital. Definitive radiation therapy consisted of external beam radiation therapy (50Gy to whole pelvis, central shielding after 40Gy/fraction 2Gy) and brachytherapy 7Gy, 3 fractions were given in 59 patients. Concurrent chemoradiotherapy (CCRT) with weekly cisplatin 40mg/m2, 5 cycles concurrent with RT were given in 14 patients. Treatment toxicity and efficacy (overall survival) were analyzed. Result: All patients completed treatment. The 4-years overall survival rate was 56.4%. Acute diarrhea, hematological, urinary toxicity incidences were 27.6%, 31.5% and 9.6%. Long-term toxicity rate was 11.2%, included hemorrhagic cystitis, hemorrhagic proctitis and chronic diarrhea. Conclusion: Definitive treatment in elderly cervical cancer patients was safe and effective.
