Evaluation of the results of surgical treatment of cryptotia by the modified multiple V-Y advancement flap

Abstract

Objective: V-Y plasty technique has been applied on cases of cryptotia. Despite its many advantages, the disadvantages of the technique are the development of a visible scar and a lowered hairline. The study was aim to evaluate the results of surgical treatment of cryptotia by the modified multiple V-Y advancement flap. Subject and method: Research objectives consist of six patients with nine cases of cryptoria who were adopted the multiple V-Y advancement flap modification technique at the Centre for Craniofacial and Plastic Surgery - 108 Military Central Hospital, from January 2018 to December 2021. The follow-up period ranged from six months to four years. Result: Six patients (including four male patients and two female patients) had an average age of 6.67 years with the youngest age was 3, and the oldest age was 15. The auriculocephalic sulcus was reconstructed. The auricular angle of the mastoid was increased from 10 degrees to 12 degrees. Treatment results were good at 55.6%, neutral at 33.3%, and poor at 11.1%. One patient was suffered from keloid scars. The scars were hidden behind the earslobe, and the hairline was not lowering. No case of recurrence was reported. Conclusion: The multiple V-Y advancement flap modification technique has been effectively applied on all cases of cryptotia. With a high success rate, it overcome problems of the classic V-Y techniques. This is a good choice for surgeons in treating the disease.
