Abstract

Objective: To describe some clinical characteristics and evaluate the nursing care of patients with primary knee osteoarthritis in Rheumatology Department of 108 Military Central Hospital. Subject and method: Prospective and descriptive study in 245 patients with primary knee osteoarthritis treated at Rheumatology Department of 108 Military Central Hospital from 11/2020 to 05/2021. The researched index included: Visual Analogue Scale (VAS) from 0 - 100mm, the range of motion (flexion/extension). The evaluation of result of patients education in term of nursing care based on the Likert classification with 5 levels from “non-education” to “comprehension and good performance”. The evaluation of satisfaction of patients in term of nursing care based on the Likert classification with 5 levels from “deep unsatisfaction” to “high satisfaction”. The evaluation of nursing care was classified into 5 levels (worst to excellent) based on 6 criteria proposed by Ministry of Health, each criteria was scored from 1-5 points, and general point > 15 was considered as achieved. Times of study included T0 admission, T1 after admission 3 days, and T2 after admission 7 days. Statistic analysis was performed by SPSS version 16.0, continous variables were presented with  ± SD, comparison between each time points by Ttest, statistical significance if p<0.05. Result: The average age was 62.29 ± 13.01 years old; the manual occupation contributed to 26.53%; both knees osteoarthritis accounted for 59.59%. The proportions of patients with morning stiffness and mechanical pain were 88.16%. The average of VAS score at T0 was 5.85 ± 2.55 points and T2 was 2.15 ± 1.12 points. The range motions were progressed from T0 121.45 ± 7.290 to T2 132.56 ± 6.520 with flexion; and from T0 23.46 ± 12.450 to T2 8.24 ± 3.230 with extension. The results of patients educations generally stood at level 4 (comprehension and ability of performance) (51.49%). The proportion of patients without being educated was 8.94%; and the percentage of educated patients without comprehension was 5.11%. A total of 86.53% patients were satisfied and more in term of nursing care, and there were 2.04% patients unsatisfied with nursing care, especially in criteria of “attitude of service, patient education and follow-up”. The nursing care at non-achieved level was 3.2%; the most problematic was “daily evaluation of pain intensity” (2.85% non-achieved). Conclusion: The average age of studied population was 62.29 ± 13.01 years old; female/male was 1.5/1. The proportion of patients in “comprehensive and ability of performance” after nursing consultation and care was 51.49%. A total of 86.53% patients was satisfied with nursing care. Achieved level in nursing care contributed to 96.7%.
