Đặt vấn đề: Gãy xương đốt sống là một biến chứng nghiêm trọng của loãng xương, gây tàn phế và làm giảm chất lượng cuộc sống ở người cao tuổi. Đái tháo đường típ 2 đã được chứng minh là tình trạng làm tăng nguy cơ gãy xương thông qua các rối loạn chuyển hóa xương ở cấp độ tế bào. Ở phụ nữ cao tuổi, sự phối hợp của đái tháo đường típ 2 và loãng xương có thể làm gia tăng nguy cơ gãy xương đốt sống. Tuy nhiên, tại Việt Nam, dữ liệu về vấn đề này vẫn còn hạn chế. Mục tiêu: Xác định tỷ lệ, đặc điểm gãy xương đốt sống trên phim X-quang và các yếu tố nguy cơ dự đoán gãy xương đốt sống ở phụ nữ  trên 65 tuổi mắc đái tháo đường típ 2 kèm loãng xương. Đối tượng và phương pháp: Nghiên cứu cắt ngang được thực hiện trên 98 phụ nữ trên 65 tuổi mắc đái tháo đường típ 2 kèm loãng xương, khám tại Khoa Nội tiết và Khoa Cơ xương khớp, Bệnh viện Nguyễn Tri Phương, từ tháng 10/2024 đến tháng 06/2025. Kết quả: Tỷ lệ gãy xương đốt sống là 14,3%. Đa số bệnh nhân gãy tại một đốt sống; vị trí thân sống T12 là vị trí thường gặp nhất. Kiểu gãy chủ yếu là gãy hình chêm (81,25%) và gãy mức độ nặng chiếm 50%. Hai yếu tố nguy cơ độc lập liên quan đến gãy xương đốt sống là thời gian mắc đái tháo đường (OR = 3,9; p=0,008) và biến chứng võng mạc do đái tháo đường (OR = 7,3; p=0,006). Kết luận: Phụ nữ cao tuổi mắc đái tháo đường típ 2 có tỷ lệ gãy xương đốt sống đáng kể, đặc biệt ở những đối tượng có biến chứng võng mạc và thời gian mắc đái tháo đường kéo dài.
