Tóm tắt

Mục tiêu: Đánh giá chi phí - hiệu quả của phác đồ ribociclib phối hợp với thuốc ức chế aromatase không steroid (NSAI) + goserelin so với NSAI + goserelin trong điều trị ung thư vú giai đoạn tiến xa, di căn HR+/HER2- ở phụ nữ chưa mãn kinh tại Việt Nam. Đối tượng và Phương pháp: Nghiên cứu phân tích chi phí - hiệu quả trên quần thể giả định gồm phụ nữ chưa mãn kinh mắc ung thư vú tiến xa, di căn có HR+/HER2-. Mô hình sống còn dịch chuyển từng phần với ba trạng thái (không tiến triển, tiến triển, tử vong) được sử dụng, với chu kỳ 28 ngày, khung thời gian trọn đời và quan điểm cơ quan chi trả bảo hiểm y tế (BHYT). Các tham số đầu vào cho mô hình được thu thập từ dữ liệu y văn đã công bố và tham vấn ý kiến chuyên gia. Kết quả: So với NSAI + goserelin, bổ sung ribociclib giúp tăng thêm 1,44 QALYs với chi phí tăng thêm 630 triệu đồng, tương ứng ICER 438 triệu đồng/QALY (≈3,8 lần GDP/người năm 2024). Nếu BHYT chi trả 80%, 70% và 60%, ICER giảm còn 363; 325 và 288 triệu đồng/QALY (tương ứng 3,2; 2,9 và 2,5 lần GDP/người). Phân tích độ nhạy xác suất cho thấy với ngưỡng chi trả ≥ 433 triệu đồng/QALY, xác suất chi phí - hiệu quả vượt 50%. Kết luận: Phác đồ ribociclib phối hợp letrozole và goserelin đạt ICER là 438 triệu đồng/QALY. Trong trường hợp BHYT đồng chi trả ở mức 60 - 80%, ICER giảm xuống 2,5 - 3,2 lần GDP/người - tiệm cận các ngưỡng chấp nhận chi trả tại Việt Nam. Do đó, ribociclib có thể được xem xét như một lựa chọn chi phí - hiệu quả có điều kiện khi áp dụng cơ chế đồng chi trả.
