Phẫu thuật nội soi xuyên chân bướm xử trí rò dịch não tủy ở ngách bên xoang bướm tại Bệnh viện Tai Mũi Họng Trung ương: Báo cáo chùm ca bệnh

Mục tiêu: báo cáo chùm 03 ca bệnh rò dịch não tủy qua ngách bên xoang bướm được phẫu thuật nội soi xuyên chân bướm bít lấp đường rò tại Bệnh viện Tai Mũi Họng Trung ương. Đối tượng và phương pháp: báo cáo chùm ca bệnh. Kết quả: Cả 3 ca bệnh đều có quá khí hóa xoang bướm ở cả cánh lớn và chân xương bướm, được phẫu thuật nội soi xuyên chân bướm để tiếp cận vị trí rò, sử dụng vật liệu nhiều lớp bao gồm cân cơ đùi, mảnh ghép sụn, vạt niêm mạc vách ngăn, sau đó gia cố bằng keo sinh học, cố định bằng surgicel và merocel mũi. Sau mổ được chăm sóc theo quy trình chung của bệnh nhân phẫu thuật rò dịch não tủy qua mũi. Kết quả cho thấy cả 3 ca đều thành công, không để lại di chứng, không tái phát với thời gian theo dõi 15 - 18 tháng. Kết luận: Phẫu thuật nội soi xuyên chân bướm là phương pháp an toàn, cho thấy tính khả thi và kết quả bước đầu khả quan trong điều trị rò dịch não tủy qua ngách bên xoang bướm, đây là kỹ thuật đã được thực hiện thành công tại Bệnh viện Tai Mũi Họng Trung ương.
