Nghiên cứu giá trị của thang điểm ABC trong tiên lượng chảy máu tiêu hóa do vỡ giãn tĩnh mạch thực quản, tĩnh mạch phình vị ở bệnh nhân xơ gan

Mục tiêu: Đánh giá giá trị của thang điểm ABC trong tiên lượng tái chảy máu sớm và tử vong sớm của chảy máu tiêu hóa (CMTH) do vỡ giãn tĩnh mạch thực quản (TMTQ), tĩnh mạch phình vị (TMPV) ở bệnh nhân xơ gan. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang, không nhóm chứng, hồi cứu kết hợp tiến cứu trên 201 bệnh nhân xơ gan có biến chứng CMTH do vỡ giãn TMTQ, TMPV điều trị tại Viện Điều trị các bệnh Tiêu hóa - Bệnh viện TƯQĐ 108 từ tháng 9/2022 đến tháng 4/2023. Tính điểm ABC và đối chiếu với tỷ lệ tái chảy máu sớm và tử vong sớm. Kết quả: Thang điểm ABC có giá trị tốt trong tiên lượng tử vong sớm với diện tích dưới đường cong (AUROC) 0,804; 95% CI: 0,70-0,91; p<0,001; tại điểm cắt 8 có độ nhạy 81% và độ đặc hiệu 79,4%. Có giá trị tiên lượng khá đối với tái chảy máu sớm với AUROC 0,744; 95% CI: 0,59-0,89, p=0,002. Tại điểm cắt 9 có độ nhạy 62,5% và độ đặc hiệu 84,9%. Kết luận: ABC là thang điểm có giá trị trong tiên lượng nguy cơ tái chảy máu và tử vong sớm ở bệnh nhân xơ gan có biến chứng CMTH do vỡ giãn TMTQ, TMPV.
